इस महाविद्यालय की स्थापना वर्ष 1965 में हुई। उस समय पूरे क्षेत्र में उच्च शिक्षा की कोई ब्यवस्था नहीं थी । उच्च शिक्षा के लिए शहर की ओर जाने के अलावा छा़त्रों के लिए कोई विकल्प नहीं था फलतः बहुत कम लोग ही उच्च शिक्षा प्राप्त कर पाते थे। क्षेत्र में उच्च शिक्षा की महती आवश्यकता को देखते हुए क्षेत्र के प्रतिष्ठित लोगों ने एक डिग्री कालेज स्थापित करने का बीड़ा उठाया और श्री शिवा डिग्री कालेज, तेरही, कप्तानगंज, आजमगढ़ के रुप में उनका सपना साकार हुआ। इस कार्य में श्री शिवा डिग्री कालेज, एजूकेशनल सोसाइटी, तेरही, कप्तानगंज, आजमगढ़ के अघ्यक्ष स्व0 मुक्ति नाथ राय, प्रबंधक विष्णु दत्त मिश्र तथा सहायक प्रबंधक देवता पाण्डेय की भूमिका सराहनीय रही । अपने अनुशासन एवं शिक्षा की गुणवत्ता के कारण जिले में इस महाविद्यालय का विशेष स्थान है | महाविद्यालय श्री वीर बहादुर सिंह पूर्वांचल विश्वविद्यालय से सम्बद्ध है | यह महाविद्यालय श्री शिवा डिग्री कालेज एजुकेशन एसोसिएशन तेरही द्वारा संचालित है |

Providing a friendly, inclusive, learning environment where all take pride in learning and achievement.
Contributing to reductions in illeteracy and unemployment in boys and girls especially in the underprivileged populations living in the rural areas for improving the quality of life and socio economic development.
Mission
The mission of this college is to prepare the students to become knowledgeable, contributing citizens in a country of diverse cultures. Vital to the mission of this educational group is the discovery of new knowledge through teaching and learning, research and creative activity. The role of this group is to nurture and sustain the learning and understanding achieved.
Immediate Goals
- Designing of curriculum for UG and PG Courses. As per University norms
- Publication of calendar of events and conduct of classes, examinations and all other activities strictly as per the calendar.
- Review of academic programmes from time to time.
- Starting and conducting new academic programmes in thrust areas.
